Quick answer

  • The UK does not require travel insurance for US visitors.
  • The NHS is not free for tourists. Under the NHS charging regulations, overseas visitors are generally billed at 150% of the NHS national tariff for hospital treatment.
  • Some care is free for everyone, including treatment in A&E (emergency department) itself, but once you are admitted as an inpatient, charges usually apply.
  • Hospitals may ask for payment while you are still a patient, and if you have travel insurance you may need to pay first and claim later.

Many Americans assume the UK’s National Health Service treats everyone for free. It does not. Visitors from outside the UK are generally charged, and billed at more than the NHS’s internal rate. This guide explains how charging works, what is free and what coverage makes sense for a London trip or a tour of Scotland.

Is travel insurance required to visit the UK?

No. US citizens can visit the UK as tourists without a visa, but they now need an Electronic Travel Authorisation (ETA) before travel. The ETA does not require insurance. The UK government still advises all visitors to have travel or health insurance.

How the NHS charges visitors

The National Health Service (Charges to Overseas Visitors) Regulations require NHS hospitals in England to identify overseas visitors and charge them for most hospital care. For visitors from countries without a reciprocal agreement, including the United States, the charge is generally 150% of the national NHS tariff. Scotland, Wales and Northern Ireland run their own versions of these rules.

Practical points:

  • Hospitals must raise an invoice regardless of your ability to pay, and may ask for payment before or during treatment for non-urgent care.
  • Insurance is usually reimbursed later. Several NHS trusts state that overseas visitors with travel insurance must pay and then claim back from their insurer.
  • Unpaid NHS debts above a certain amount can affect future UK visa or entry applications.

What is free for visitors?

Some services are exempt from charges for everyone, including:

  • Treatment in an A&E department or urgent treatment center, up to the point you are admitted to a ward or referred to an outpatient clinic.
  • Treatment of certain infectious diseases listed in the regulations.
  • Family planning services (not including termination of pregnancy).

GP (family doctor) visits are handled differently, and prescriptions, dental treatment and ambulance-related costs can still create bills. Private hospitals and clinics charge full private rates.

Renting a car in the UK

Your US auto policy generally does not cover you in the UK. Rental rates usually include the legally required third-party liability, but collision damage waivers and excess (deductible) cover are extra. Some US credit cards cover rental car damage in the UK; check the terms, and remember that driving on the left increases the chance of minor scrapes.

Medicare and US plans

Original Medicare generally pays nothing in the UK. Medigap plans with foreign travel emergency coverage pay 80% after a $250 deductible, up to $50,000 lifetime, with no evacuation benefit. Employer plans often cover emergencies abroad as out-of-network care, but rarely cover repatriation.

Frequently asked questions

Is NHS treatment free for American tourists?

Mostly no. Overseas visitors are generally charged 150% of the NHS national tariff for hospital care. Treatment in A&E itself and some other services are free until you are admitted or referred.

Do I need travel insurance to visit London?

It is not required, but it is strongly recommended by the UK government because NHS hospital care is chargeable for visitors and private care is expensive.

Will the NHS bill my travel insurer directly?

Often not. Many NHS trusts require overseas visitors to pay and then claim the cost back from their insurer.
